- Consent forms and forms associated with your cataract surgery
- Scheduling for cataract surgery (performed on one eye at a time, if you are having cataract surgery in both eyes, you will have two surgery dates)
- Instructions to prepare for cataract surgery
- Instructions for post-operative care
